Person Centred Care
Person centred care starts with the person rather than the task list. We ask how someone likes their day to run, what they still want to do for themselves and where a little help would make the difference, then build the support around that.

- A plan written with the person, in their own words
- Support agreed around preferences, routines and interests

All support is subject to assessment and availability, and is agreed in writing before it begins.
