Supported Living
Supported living helps adults live in their own home with the level of support they choose. The focus is independence: building skills, keeping routines and being part of the local community, with support that can flex as things change.

Who this may suit
- Adults with a learning disability living in their own tenancy
- People with a physical disability who want support on their own terms
- Someone moving towards greater independence from family or residential care
- Anyone whose support needs may change over time
What support may include
- Support with daily living skills such as cooking, budgeting and laundry
- Help managing appointments, post and household admin
- Encouragement with education, volunteering, work and social plans
- Getting out and about, and building travel confidence
- Goal setting agreed with the people who matter
- Working alongside families and professionals, with consent
All support is subject to assessment and availability, and is agreed in writing before it begins.

After an assessment we write a clear plan together and agree who visits, when and what support may include.

Care begins with a familiar team, and we review it regularly so the plan keeps pace with any change.
